Antibiotic resistance is a real problem in healthcare, and as providers, we need to do a better job promoting good antimicrobial therapy stewardship. Not all positive cultures require treatment (ie, colonizers). Not every "empiric" regimen needs to be continued indefinitely. We need to be more mindful of clinical signs of infection and knowing when to peel off antibiotics using the patient's overall symptoms, biomarkers (like procalcitonin), and seeking the expert advice from our friends in the infectious disease (ID) department.
