STS Short-Term Operative Risk Scoring System

As with any procedure, cardiac surgery carries risks that are explained to patients by their surgical team. The Society of Thoracic Surgeons Short-Term Operative Risk Calculator provides clinicians with a way to anchor these discussions. It uses data from hundreds of thousands of cardiac operations to estimate the chance of outcomes like mortality, stroke, renal failure, prolonged ventilation, reoperation, and significant morbidity during the same hospitalization.

Like everything else in medicine, these estimates are never perfect. Rapidly changing physiology, frailty, and many of the subtle bedside factors that influence outcomes are not captured by the calculator. I see it as a starting point rather than a final answer. It gives structure to how we think about risk, helps us anticipate resource needs, and often provides insight into what can be optimized before surgery.

Most importantly, the calculator helps everyone speak the same language. Saying a patient has a 5% predicted risk of significant morbidity is far clearer than calling them “moderate risk.” Overall, this calculator is another tool to help us care for cardiac surgical patients more effectively.
